In the previous episode, “Respect the Process. Respect the Quirks,” Watson realizes that brain damage has caused him to hallucinate Sherlock Holmes. But before he could act on it, he was notified about a new case. At the clinic, a mother, Shelly, brings her daughter, Cora, claiming the girl has cancer.

After a series of events, Watson and his team discover that Shelly has a mental disorder, which has driven her and her daughters to extreme, isolated living conditions. While Watson and Laila are returning from Shelly’s home, he hallucinates Holmes once again, leading to a car accident.
